This is an exciting Pharmacist opportunity in Cheadle, focused on specialist home‑care pharmacy therapies.
Essential Duties & Responsibilities
* Act as the Responsible Pharmacist to deliver prescription management and dispensing across all therapies in line with operational KPIs and standards, while maintaining patient safety.
* Process prescription receipts and conduct clinical screening/intervention in line with the standard operating procedures.
* Accurately dispense prescriptions and ensure the timely reporting of near‑misses and dispensing errors.
* Place product orders and authorise prescription entries on the Patient Medication Record.
* Formulate unlicensed medicines and raise purchase orders with the manufacturing provider in line with patient and customer requirements within contractual timeframes.
* Communicate and collaborate with relevant clinical teams to support patient adherence and elevate concerns where appropriate.
* Record and elevate adverse events and incidents to the appropriate escalation points (e.g., Lead Pharmacist, Quality, Pharmacovigilance, Superintendent Pharmacist, Safeguarding Lead).
* Provide clinical counselling and product expertise for patients, associated care teams, and pharmacy support staff.
* Mentor and coach pharmacy support staff in the delivery of associated pharmacy training.
Education and Experience
* Registered Pharmacist with the GPhC.
* Post‑registration experience in a home‑care pharmacy setting is advantageous.
* Successful experience in undertaking clinical investigations such as near‑miss investigations.
* Strong coaching and training capabilities, with the ability to motivate and develop others.
* Critical thinker with strong problem‑solving and analytical skills.
* Maintain up‑to‑date knowledge of GPhC and RPS pharmacy standards as part of continuous professional development (CPD).
