Care Coordinator — Early Intervention
Band 6 – fixed‑term, 6 months maternity cover only, secondment available. One full time post and one part time post (19 hrs per week). Please state preference in application.
Hours: Full time (37.5 hours per week, Monday to Friday 9am‑5pm) or Part time (19 hrs per week).
Job Overview
We are looking for a Nurse, Social Worker or Occupational Therapist to join the Early Intervention Psychosis team in Peterborough (CAMEO).
The role of Care Coordinator is to work in partnership with CAMEO service users, developing their care plans and recovery plans, implementing interventions, and providing specialist advice. The post holder will promote the safety and wellbeing of service users, assess, plan and implement care, and provide clinical supervision to staff and students. The early intervention approach and ethos are to be adhered to, including work with under‑17 age range when required.
Key Responsibilities
* Assess individuals and their families/carers with complex health presentations, including those presenting with higher risk.
* Develop care plans and recovery plans with service users.
* Provide specialist professional assessment and advice.
* Assess and manage ongoing risks, collaborating with service users and carers.
* Regularly review risk factors and adjust management strategies.
* Travel independently around the county; hold full UK driving licence and vehicle access.
Person Specification
Education and Qualifications
* Registered Nurse (mental health/LD/Adult) or Registered Social Worker or Registered Occupational Therapist.
* Evidence of continuous professional development.
Experience
* Significant post‑qualifying experience with serious mental illness.
* Experience working with complex health problems in the designated care group.
* Experience managing a caseload.
* Experience providing a range of clinical interventions for this care group.
* Experience conducting clinical assessments including risk assessments.
* Experience and knowledge of dual diagnosis/substance misuse.
* Previous experience working in the community.
* Lived experience of mental health issues.
Skills & Abilities
* Excellent written and oral communication skills.
* Ability to develop effective professional relationships.
* Proactively develop own skills and provide information to others.
* Assess and interpret service user conditions with appropriate actions.
* Plan and organise complex activities or programmes.
* Proficient in Word, PowerPoint, and clinical systems.
* Ability to multitask and adapt mentally to unpredictable situations.
Other
* Ability to travel independently around Cambridgeshire and Peterborough area, meeting required timescales.
* Punctual and flexible across working hours when required.
* Demonstrate support for the values and beliefs of the Care Group and the Trust, particularly for young people and families.
Physical Requirements
* Use manual handling techniques.
* Energy to work with younger and older people.
* Monitor and maintain health and safety of self and others.
* Required to use a PC.
