Public Health Practitioner – Community Health
Nottinghamshire Healthcare NHS Foundation Trust invites applications for a qualified registered nurse to join the Ashfield Healthy Family Teams. The post holder will work across community settings, health centres, schools, children’s centres and client homes to deliver the Universal Healthy Child Programme and Tier 1 targeted health services to the local 0‑19 population.
Responsibilities
* Support day‑to‑day management of a defined 5‑19 year workload within a skilled, mixed team.
* Deliver public health, universal and targeted interventions, including safeguarding, across the 0‑19 year age range in line with the Healthy Child Programme.
* Clinically supervise Assistant Public Health Practitioners, Healthy Child Assistants and Screening Assistants allocated to a defined geographical population.
* Work proactively with children, young people and families to promote health and address health inequalities.
* Plan, organise and implement own continual professional development and document progress.
Qualifications and Experience
* Registered nurse with current valid registration.
* Experience working in a health or community setting with knowledge of health inequalities and the public health agenda.
* Experience in care planning, delivery and evaluation of interventions.
* Accountable for own reflective practice learning and maintaining competence through mandatory training, including safeguarding.
* Knowledge of the 0‑19 Healthy Child and Public Nursing Programme and key performance indicators.
* Ability to work in agreed areas of practice without direct supervision, recognising boundaries of practice.
Other Requirements
* Full UK driving licence and vehicle for business use.
* Ability to travel across Trust sites.
* Completion of a DBS check; costs (£26.40 standard or £54.40 enhanced) will be deducted from salary over the first two months of employment.
* Encouraged to enrol in the DBS Update Service – annual fee £16.

Contract and Salary
* Band 5 – Fixed term, 12‑month contract.
* Salary £32,073 to £39,043 per annum (pro rata for part‑time).
