ACT (Accessing Change Together) integrates council staff and partner organisations and provides a range of support and interventions in relation to service users who need support with Housing Support and Homelessness, Domestic Abuse and Substance Use.
ACT (Accessing Change Together) is an integrated service, which means that lots of support services are working together and using a shared case load management system. This makes it easier for service users to get access to services they need so they receive the right support and easier for council staff and partner organisations to work together. The teams work alongside other services including Adult and Children’s Social Care and Public Health, to develop consistent pathways and effective processes.
We have an opportunity available within ACT (Accessing Change Together) for a team manager to oversee the Community Interventions Team in Middlesbrough council. This exciting new role, which is funded by the Changing Futures programme will support and assess prison leavers returning to the South Tees area and working on behalf of Middlesbrough and Redcar and Cleveland’s statutory Homelessness Services.
The role will be based within Middlesbrough Council’s Housing Solutions Team. The role will focus on assessing prisoners in a timely manner to explore their housing options thoroughly in advance of their release. The role is expected to have a significant impact in the reduction of Rough Sleeping across South Tees, by ensuring local pathways are followed. The role will be based across Local Authority buildings and within local prison settings to carry out face to face assessments.
Whilst in post there will be development opportunities alongside flexible working with the option of a blended working environment (home and office where appropriate).

To be successful in this role you need to demonstrate how they prioritise development and well-being of a team, has experience of working effectively with a range of services and has the skills, passion and resilience to work creatively with service users who are accessing ACT accessing Change Together and need ongoing support in relation to Housing, Homelessness and /or Domestic Abuse.
Please note, this is a fixed term vacancy until 31st December 2026.
