Tuxford Medical Centre is seeking to appoint an additional member of staff to compliment their existing non-clinical team.
The ideal candidate will be able to work in a fast paced environment and be able to adapt to change easily. You will need to have a strong work ethic and be able to work to tight deadlines. The role involves patient contact both in person and on the phone, so good communication skills are required.
Main duties of the job
Our Care Coordinators provide extra time, capacity andexpertise to support patients in preparing for (or in following-up) clinicalconversations they have with primary care professionals. They will work closelywith the GPs and other primary care professionals to manage patients, makingsure that appropriate support is made available to them and their carers, andensuring that their changing needs are addressed.
Care Navigators ensure that the Practice runssmoothly and assist with other back office administration. This role provides ahigh level of service to patients, doctors, nurses and other members of thePractice team.

To ensure that the reception area of the Practice runs smoothly and to assist with back office administration of medical records. This role provides a high level of service to patients, doctors, nurses and other members of the Practice team.
Job Responsibilities:
Telephone Duties
* Giving out results
* Ambulance transport bookings
* Home visit requests
* Deal with general enquiries and complaints
* Taking requests for prescriptions
* Contacting other providers
Reception Duties
* Register new patients and temporary residents
* Greet and direct patients and visitors
* Taking triage information for GP appointments
* Patient Care Navigation
* Handing out prescriptions
* Ensure outstanding queries are explained and handed over to next shift, as necessary
* Respond to needs of doctors and nurses during surgery
* Collect payments from patients for non-NHS services
* Deal with general enquiries and complaints
Administrative Duties
Culling and sorting patient records and letters in preparation for summarising
Reviewing medical records and producing an accurate summary of the patients medical history
Read coding the information into the practice clinical system
o Auditing data collection standards in the practice
o Monitoring progress of notes summarising against practice targets
o Monitoring patient call and recall systems
Pulling Lloyd George envelopes when requested by doctors
Other
General housekeeping (e.g. keeping reception and waiting areas tidy)
Make refreshments for doctors and other team members, participate in washing up
Any other reasonable duties as necessary

Experience
* Good level of IT literacy
* Previous experience of working in a GP Practice
