Support Worker Location: Shrewsbury Hours: 20 hours per week Salary: £24,479 per annum pro rata (£13.45 per hour) Contract: Permanent Additional Requirement: Full UK manual driving licence held for a minimum of 2 years and a willingness to drive service vehicles We are looking for a dedicated Support Worker to join our 5 - day hospital discharge service in Shrewsbury. You'll work closely with hospital teams and partner organisations to help people return home safely after a hospital stay and maintain their independence. What does a day in the life of a support worker look like? Respond to referrals and complete welfare calls and home visits. Support return home from hospital Support after discharge from hospitals Provide practical and emotional support to aid recovery and independence. Assess needs, identify safeguarding concerns, and accurately record case notes. Signpost individuals to appropriate services and community resources. Instillation of home safety alarms and key pendant boxes to offer further security. To be a successful Support Worker, what will you need? A kind, calm and confident approach with great people skills. Right to Work in the UK. Unfortunately we are unable to offer sponsorship for this role. The ability to work independently and as part of a team. A full UK driving licence and willingness to drive service vehicles. Basic IT skills and a commitment to following procedures. This is a hands-on, community-based role where no two days are the same. You'll need flexibility and confidence working in different locations across Sandwell. Interested?
