Job Overview
This post involves working in outpatient clinics providing rapid access, admission avoidance clinics for the frail population of Hillingdon and general elderly medicine clinics.
Working Hours
The current set up of this post is for 2 full days of clinics and SPA worked flexibly, suitable for less than full time or flexible working.
Responsibilities
* Provide rapid assessments in Rapid Access Clinics for the Elderly (RACE) up to five times a week, assessing community-dwelling frail patients referred by GPs.
* Follow up patients seen by our service on the wards or in the Acute Frailty Unit supporting ED.
* Attend general elderly medicine clinics, delivering a holistic approach to care for patients over 65 and managing a wide range of presentations.
* Provide seven‑day a week frailty SDEC services as part of our expanded acute frailty provision.
Salary and Benefits
£113,565 to £150,569 per annum, plus £2,162 London Weighting.
