About UsThe Home First Neighbourhood City Team is part of the Home First service in Oxfordshire County Council Adult Social Care. The Home First service has revolutionised the way OCC supports people with lived experience of hospital admission return home to receive the right level of care in the right environment at the right time. We are a highly-collaborative reablement service that works in close partnership with Oxfordshire's NHS health trusts and many other partner agencies. The Home First Neighbourhood City Team supports the residents of Oxfordshire in having their needs assessed and met in their own homes, rather than at a hospital bed side.About the RoleWork location: John Radcliffe Hospital, with agile working across Oxford City (OX1-4)A combination of remote, face to face and office-based working is expected depending on the needs of the peoplesupported.Hours per week:37with rotational weekend work (max. 21 weekend days per 12 months)Contract type:PermanentSalary range: Grade 8/9 £31537 - £37280 per annum plus a 5% enhancement for weekend working pattern.Vetting requirements: This role is subject to an Enhanced DBS with Adults Disbarring ListThis is open to existing OCC employees who are looking for a new opportunity and to external applicants interested in joining OCC.As part of our team you will:Work closely with our reablement providers and the voluntary sector you will support and guide people to regain independence in their own home using a strength-based approach to goal planning during short term intermediate care provision. This intervention will be provided following discharge from hospital or to help prevent admission to hospital.Draw upon your skills and knowledge in activity analysis, assistive technology, aids and minor adaptations to support the reduction in formal care wherever possible.
