We are looking for a District Nurse / Community Sister/Charge Nurse to join our multidisciplinary community team. This includes coordinating and delegating individual patient care by working across boundaries within secondary and social care settings. The role involves managing a caseload, providing clinical case management for patients with complex health needs at risk of deterioration, which could lead to a decline in quality of life, avoidable hospital admissions, or unnecessary hospital stays.

Main duties of the job
As a District Nurse / Community Sister / Charge Nurse, your main duties will include (but are not limited to):
1. Accountable for a patient caseload and delegating responsibilities within the multidisciplinary team, in conjunction with other key professionals.
2. Applying advanced assessment skills of physical, mental, and social care needs to provide clinical care and health-promoting interventions to help avoid unnecessary hospital admissions.
3. Interpreting clinical signs, symptoms, and diagnostic tests to develop treatment plans in collaboration with medical practitioners, providing advanced clinical advice to improve patient outcomes.
4. Facilitating patient independence through self-management strategies.
5. Developing and analyzing audits, interpreting data, and using findings to enhance clinical practice. Please see the attached job description for full details.

Job responsibilities
Your key responsibilities will include (but not limited to):
* Managing a designated patient caseload and delegating care responsibilities within the team.
* Supporting the team leader and clinical lead in providing, monitoring, and improving care quality, including risk management, CQC compliance, and clinical audits.
* Overseeing clinical areas and facilitating junior staff leadership.
* Deputising for the Clinical Nurse Lead when necessary, assisting in staff recruitment, appraisals, risk assessments, complaints, and performance management.
* Providing clinical supervision, peer review, leadership, and mentorship to junior colleagues.
* Being aware of the Trust's financial position and managing costs related to equipment and supplies, ensuring cost-effective care and proper management of clinical stock and equipment maintenance.
* Reviewing practice roles and participating in appraisals to identify development needs and personal development plans.
* Acting as a practice assessor for staff and students.
Person Specification
* Registered General Nurse
* Post-registration study
* Community Specialist Practice qualification
* Experience in Case Management
* Minimum 2 years community experience
* 4 years post-registration experience
Skills and Knowledge
* Range of clinical skills suitable for community nursing
* Experience of Case Management
