About the Role: As a Care Coordinator, you will play a vital role in matching clients with suitable care providers, ensuring the highest quality of care is delivered.
Key Responsibilities: Matching clients with appropriate care providers based on their needs and preferences.
Coordinating care schedules and ensuring optimal coverage.
Building strong relationships with clients and care providers.
Providing excellent customer service and support.
Managing and updating client records and care plans.
Handling administrative tasks, including scheduling, documentation, and reporting.
Collaborating with the clinical team to ensure seamless care delivery.
Empathy and compassion for elderly and vulnerable individuals.
Skills and Qualifications: Essential: SVQ Level 3 in Health and Social Care or equivalent.
Proven experience in care coordination or a related field.
Excellent organizational and time management skills.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Knowledge of care standards and regulations.
Proficiency in using computer systems and software.
