This is an exciting opportunity for a community mental health nurse who has some experience of working with older adults and caring for individuals with Enduring Mental Health problems.

The post will include care coordinating a designated caseload, working within CPA Policy and associated countywide risk procedures, travelling independently, use of IT skills and a working knowledge of legislation regarding safeguarding vulnerable adults, the MHA and MCA.

Assess, plan, implement, evaluate the nursing care needs of a specific group of service users within the community setting

Supervise mentor, teach qualified & unqualified staff including students

Co-ordinate care for a specified group of service users

Applicants Will Have

Community experience/equivalent within mental health services.

Be self-motivated, organised, able to effectively manage, care coordinate an active caseload.

Excellent communication & problem-solving skills.

Have a flexible & creative approach

Able to work autonomously & as part of a team (will be subject to lone working).

Show evidence of continuous professional development.

Able to transport self around the locality & undertake indicated statutory & mandatory training.
