The Community Learning Disability Team (CLDT) in North Tyneside is part of the Northumbria Healthcare NHS Foundation Trust and operates as a multi‑disciplinary team in partnership with the local authority.
The role will facilitate the provision of specialist services for children and adults, offering assessment, intervention, and training for people who have a learning disability, complex behavioural or mental health needs.

Responsibilities
* Support the senior nurse with monitoring of behavioural caseload, assessments, interventions, and risk management in the community.
* Assist senior staff in developing highly specialised Positive Behavioural Support Plans for patients and service providers to reduce placement breakdowns or admissions to inpatient settings.
* Provide and support highly specialised behavioural assessments and interventions for children and adults presenting with severe behaviours that challenge, within the Enhanced Crisis Aversion Team.
* Carry out behavioural assessments, observations and interventions under the direction of a senior nurse, and contribute to the development of nursing care and behaviour support plans for local service users.
* Participate in all relevant meetings, service development activities, audits and quality improvement initiatives.
* Act as a qualified car driver.
