Overview
An exciting opportunity has arisen for an experienced HCSW to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS). The Intermediate Care Service provides rehabilitation to the adult population within the locality, delivered in patients' own homes or as out‑patients.
Responsibilities
* Work with the Physiotherapy Service within Intermediate Care to deliver therapeutic treatment programmes to service users in their own homes, preventing hospital admission and supporting early discharge from hospital.
* Assist therapists with specialist assessments and allocate work to this post holder.
* Hold own caseload, assess, progress and discharge autonomously within parameters.
* Carry out ongoing treatment programmes for a mixed caseload of mainly older people.
* Work as part of a supportive multidisciplinary team and may be required to work at other community hospital sites.
* Public holiday working is in operation.
* Be capable of travelling throughout the area to meet service user needs.
Qualifications
* HNC in Health and Social Care or equivalent or SCQF level 8 (e.g. Postural Stability Instructor or equivalent).
* Experience working in the NHS as a therapy support worker, with older people as the main group of service users referred.
* Excellent organisational and communication skills and the ability to work independently as well as within a team.
