Overview

The Christchurch Older Persons Community Mental Health Team is looking for an experienced Community Mental Health Nurse to join the team. This Mental Health Nurse role provides a comprehensive, accessible and responsive mental health service to adults and their families within a defined locality. It is primarily a care-coordination role, supporting patients on their journey of recovery. You will be a committed, dynamic, motivated professional with excellent communication and organisational skills and the ability to prioritise your work. You should have relevant and recent experience of working in a mental health environment and be able to apply the philosophies of engagement and recovery in your work with patients, their carers and their families.

You will work as part of a well-established supportive team comprising a Psychiatrist, Nurses, Psychologists, a Social Worker, Administration and a Support Time Recovery Worker. The service offers peer support led by psychology, and regular management and clinical supervision to support your practice and ongoing development.

This role is 28 hours 15 minutes per week. A valid driving licence and use of a car are required, as you will be assessing and monitoring patients in their own homes and in other community settings.

To provide a comprehensive, accessible and responsive mental health service to older adults and their families within a defined locality.

To support the team leader and the locality service manager in the implementation of the Dorset HealthCare Mental Health Strategy.

To continually develop clinical practice, team and service evaluation and delivery of service.

To contribute to effective multidisciplinary working within the community mental health service and to build effective liaison with community, primary, and other secondary and tertiary services/agencies to provide holistic care based on individual needs and service development.


Responsibilities

* Provide a comprehensive, accessible and responsive mental health service to older adults and their families within a defined locality.
* Support patients on their journey of recovery, working in a care-coordination model.
* Collaborate with a multidisciplinary team and engage with community, primary, and secondary/tertiary services to deliver holistic care.
* Assess and monitor patients in their homes and other community settings.
* Maintain high standards of clinical practice with opportunities for supervision and professional development.


Qualifications and Requirements

* Relevant and recent experience working in a mental health environment.
* Ability to apply engagement and recovery philosophies in care delivery.
* Excellent communication and organisational skills; ability to prioritise workload.
* Driving licence and use of a car for community-based visits.
* Enhanced Disclosure and Barring Service (DBS) check required; the Trust covers the cost. Applicants who subscribe to the DBS update service may present a valid DBS certificate instead of a new check.
