Occupational Therapist - Integrated Community Stroke Service

If you are a Band6 OT looking for a part-time job within the Worcestershire Stroke Pathway, wehave a post available within our Integrated Community Stroke Service whichcovers the county of Worcestershire. We are passionate about improving thequality of life for Stroke Survivors.
We would love tohear from Band 6 OTs with experience in stroke services that would like to workwithin both inpatients rehabilitation and community stroke on a rotationalbasis.

This is a part time post working 22.5 hours per week.
Main duties of the job
Day to day, you will be undertaking comprehensiveassessments of mild-moderate-severe stroke patients including those withmultiple pathologies/co-morbidities, using specialist OT assessment tools toidentify their personalised goals alongside the MDT. You will formulateindividual programmes of rehabilitation using clinical reasoning and utilisinga range of treatment skills, targeting realistic and measurable, patient-led goals.It is also expected that you will offer support and education tocarers/families.
You will attend the weekly MDT in which the team willdiscuss the progress of patients and identify goals that are achieved andongoing. As a Band 6 in the community team you will also participate inreferral management which will include the triaging of referrals, liaising withother health care professionals within the Trust/Acute Trust and surroundingborders and third-party organisations.

Job responsibilities
Provide holistic assessment andevidence-based rehabilitation to stroke patients to enable them to achievetheir best potential.
Contribute to the developmentand evaluation of the OT practice provided by the service.
To act as a source of expertiseon stroke patients management.
To maintain accuratecomprehensive and contemporaneous documentation/patient records in line withlegal requirements and Trust guidelines including personally generated clinicalassessments/treatment requiring computer skills.
To demonstrate a high level ofOT stroke skills.

Person Specification
Qualifications
* Diploma/Degree in OT
* Post graduate training relevant to the job role
* Fieldwork educator qualification within one year of appointment
* State registration with HCPC
* I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
Experience
* Experience of working with stroke patients
* Extensive clinical experience including individual and/or group work
