Cannock Villages Primary Care Network (PCN) is looking to employ a keen, enthusiastic and experienced Advanced Nurse Practitioner, to join our growing multi‑disciplinary team. They will be expected to work across a number of practices across the PCN, providing high quality, person‑centred, holistic nursing care to patients. The post holder will graduate from a Centre for Advancing Practice accredited MSc advanced practice programme and be eligible for an Advanced digital badge issued by the Centre for Advancing Practice.

Work collaboratively with the General Practitioners, and the wider Primary Care Network Team to provide expert care to patients including assessment, diagnosis, treatment and evaluation of care for patients presenting with minor illness within a Primary Care setting.
Diagnose and manage both acute and chronic conditions independently.
Prescribe medications within scope of practice and in line with local guidelines.
Contribute to health promotion, disease prevention and long‑term condition management.

It will be an opportunity to work with Allied Health Professionals to enhance the service we offer to our patients. The network consists of 11 practices with a combined list size of approx. 61k. The PCN has great access links and is located near junction 11 of the M6 motorway.

This role requires motivation and passion to deliver excellent friendly service within general practice and will be pivotal to improving the quality of care and operational efficiencies.
Job responsibilities
Primary Responsibilities
The following are the core responsibilities of the advanced nurse practitioner. There may be on occasion, a requirement to carry out other tasks as required. The role will require working at a number of different practices across the PCN but travel between practices will be limited as much as possible, implement and embed health promotion and well‑being programmes.
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
Implement and evaluate individual specialised treatment plans for chronic disease patients.
Identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects on the patients health.
Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
Undertake the collection of pathological specimens.
Prescribe and review medications in line with current guidance.
Assist GPs with the care given in local Nursing Home(s) including weekly ward rounds.
Request pathology services as necessary.
Process and interpret pathology and other test results as required.
Maintain accurate clinical records in conjunction with extant legislation.
Ensure readcodes are used effectively.
Chaperone patients where necessary.
Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
Deliver opportunistic health promotion where appropriate.
Person Specification
Qualifications
* RGN with current NMC pin or Emergency Care Practitioner
* Masters Degree - Nursing
* Independent/Supplementary Prescribing
* Graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centre's ePortfolio (Supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre for Advancing Practice, demonstrating
Experience
* Current NMC Registration.
* Eligible for an Advanced digital badge issued by the Centre for Advancing Practice.
* Experience of working in a Primary Care environment.
* Commitment to patient care.
* To adhere to the Nursing and Midwifery Council (NMC) Code of Professional Conduct and to be conversant with The Scope of Professional Practice and other NMC advisory papers.
* To use evidence‑based practice to develop and maintain high quality clinically effective and cost effective primary care.
* Experience with remote consultations and telephone triage.
