Responsibilities include, but are not limited to:
1. Undertaking assessments of Adults and Carers needs in accordance with Care Act 2014 and London Borough of Bromley procedures.
2. Preparing individual support plans reflecting the assessment of needs of the Service User and their Carers, including identifying unmet needs.
3. Arranging for the provision of standard adaptive equipment and demonstrating the correct technique for use, ensuring that the Service User and Carers are as safe and independent as possible.
4. Contributing to the development of user and carer choice and independence by effectively promoting Direct Payments/Personal budgets.
5. Working within the department's budgetary constraints to achieve best value by providing creative, cost-effective services.
6. Maintaining Adult and Carer records in accordance with departmental policy and practice.
7. Engaging with users and carers in a manner consistent with anti-discriminatory practice as developed by the Department.
8. Developing and maintaining knowledge, skills, and expertise in assessment, planning, and review.
9. Contributing to service development by providing information and ideas for future services.
Requirements:
1. Experience of working within Health and Social Care directly with Adults and Carers.
2. Evidence of continued professional and personal development.
3. Knowledge of services, resources, and procedures within Bromley to participate in the induction of new staff.
4. Ability to handle challenging family situations.
5. Thorough knowledge and understanding of the Care Act 2014, the Mental Capacity Act 2005, and other relevant legislation.
6. Experience with both short-term and long-term assessments and case management.
7. Enhanced DBS check dated within the last 12 months or willingness to complete one if not held.
