Overview

The Community Mental Health Team is seeking a skilled, motivated, organised and enthusiastic Mental Health Nurse to work within an established integrated, multi-disciplinary team, focusing upon individuals with severe and enduring mental health problems. The successful post holder will undertake responsibility for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs as defined within Parts 2 and 3 of The Mental Health (Wales) Measure 2010.


Responsibilities

* Be responsible for the care co-ordination of a designated caseload of clients with severe and enduring mental health problems.
* Ensure that Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure (Wales) 2010.
* Act as a point of contact for GPs and other interface services to offer advice, support and signposting to appropriate agencies.
* Formulate and develop standard and complex outcome-focused care and treatment plans.
* Ensure a comprehensive risk assessment is completed and documented for actual and potential risk.
* Support clinical duties such as assisting with the running of the depot/clozapine clinic and the administration and supervision of long-acting injections.
* Complete physical health assessment and side-effect monitoring for patients prescribed Clozapine (successful candidate only).
* Assess individuals in crisis, complete comprehensive assessments of health and social care needs, and agree the most appropriate course of action.
* Assess, evaluate and manage clinical risk; formulate a plan of care and interventions based on identified need and deliver evidence-based crisis interventions as appropriate.
* Work collaboratively with service users, carers and partner agencies to deliver high-quality care.


Requirements / Qualifications

* Registration as a mental health nurse and eligibility to practice.
* Ability to work within an integrated, multi-disciplinary team and to manage a defined caseload with complex needs.
* Welsh language ability is desirable; applications from English and/or Welsh speakers are welcome.
