An opportunity has arisen for an enthusiastic and motivated AHP or Nurse to join our Community-based multidisciplinary team (MDT) comprising Physiotherapists, Nurses, and Healthcare Trainers.
The successful candidate will have previous pain management experience or be able to demonstrate an interest in the specialty. They will work within the Community Pain Management Service to provide a person-centred approach to managing persistent pain. A qualification in independent non-medical prescribing is desirable but not essential.
You will work autonomously within the service, managing a caseload of patients and delivering individualised patient care. Your caseload will consist of patients with persistent pain, and you will work with them to improve their quality of life by optimising their medication. Strong communication skills and a compassionate approach to patient care are essential.
The Service works closely with the Tier 3 pain management team and the Community MSK Service. You will participate in the delivery of group pain management sessions to provide education and advice face-to-face and online.

Main duties of the job
The role will include:
* Undertaking face-to-face and telephone assessments with patients with persistent pain, producing a jointly agreed Personalised Pain Management Plan.
* Using your previous Pain Management experience to support patients in achieving their Personal Pain Management goals, on a one-to-one and group basis.
* Delivering Pain Management group sessions.
* Working with other members of the clinical team (APP Physiotherapists, Pain Specialist Nurses, Physiotherapists, and Healthcare Trainers) to support patients to live a better life with pain.
