As a city-wide Health & Wellbeing Coach within the Work Well programme, you will take a holistic and person-centred approach to supporting individuals who are out of work or at risk of leaving work due to social, physical, or mental health conditions.
Using motivational interviewing and coaching techniques, you will empower individuals with lower levels of patient activation to build the knowledge, skills, and confidence to manage their own health and wellbeing. This includes building trust, supporting self-management of health conditions, and connecting individuals with appropriate community resources and employment services.
You will provide one-to-one support to improve lifestyle, confidence, and readiness for work, working collaboratively with other services to create a single gateway to access health, employment, and community support. The role focuses on early intervention aiming to prevent long-term unemployment and promote recovery, independence, and improved quality of life.
