The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

About us
Park House Medical Centre is an NHS general practice (GP surgery) providing primary healthcare services to the local community in Prescot, Merseyside. It operates from the Prescot Primary Care Resource Centre.
* Part of the NHS primary care system, commissioned locally (Knowsley area)
* Works within a Primary Care Network (PCN) to improve population health and coordinate services
* Provides care under a General Medical Services (GMS) contract
* Around 7,800 registered patients
* Multidisciplinary team of about 23 staff, including:
o GP partners and salaried GPs
o Nurses and healthcare assistants
o Administrative and reception staff
Job responsibilities
* Make yourself available for surgery consultations, telephone consultations, home visits, checking and signing repeat prescriptions, and dealing with queries, paperwork and correspondence in a timely manner.
* Make professional, autonomous decisions in relation to presenting problems, whether self‑referral or referral from other health care workers within the organisation.
* Assess the health care needs of patients with undifferentiated and undiagnosed problems.
* Screen patients for disease risk factors and early signs of illness.
* In consultation with patients and in line with current Practice disease management protocols, develop care plans for health.
* Provide counselling and health education.
* Admit or discharge patients to and from the caseload and refer to other care providers as appropriate.
* Record clear and contemporaneous consultation notes to agreed standards.
* Collect data for audit purposes.
* Compile and issue computer-generated acute and repeat prescriptions (avoiding hand‑written prescriptions whenever possible).
* Prescribe in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.
Person Specification
Experience – Essential
* Experience of working within a General Practice team.
Qualifications – Essential
* Completion of Certificate of Training in General Practice.
UK Registration
Applicants must have current UK professional registration.
