Overview

There are development opportunities that support career progression within health visiting. We have a number of Band 7 posts that can be considered alongside either clinical or management development. BCHC also support practitioners who have been out of practice for a while, and offer a Return to Practice framework, all of the above can be discussed at interview.

The post holder is responsible for assessing need and providing health promotion and illness prevention services to the local population. The post holder will work within a corporate caseload as well as with the local population on children and family centred public health related issues. The post holder will be a first level registered nurse with a Health Visiting Degree/Diploma/Certificate. The post holder will work within the NMC Code of Professional Conduct.


Responsibilities

* You will be responsible for: Day-to-day management and delegation of work to nursery nurses, link workers and any other support staff within the team (including supervision and monitoring the work of these staff).
* Overseeing and planning in-service development for these staff.
* Be part of our team in Birmingham Community Healthcare NHS Foundation Trust (BCHC) with over 5000 staff delivering a wide range of clinical services in homes, health centres, clinics and inpatient facilities; providing care for children, young people, parents and families, adults and community services, and the wider organisation.
* Develop and maintain effective working relationships with families, primary healthcare team and other professionals in statutory and voluntary organisations.
* Deliver the Health Child Programme through compassionate assessment, analysis of need, development, implementation, and evaluation of care programmes in line with commissioner service specifications; use the common assessment framework to assess and plan with families where multiagency partnership working is required.
* Formulate and agree with families packages of care; monitor open files in accordance with records keeping policy and close when complete.
* Carry out health promotion and disease prevention activities; report on agreed key performance indicators where appropriate.
* Provide evidence-based health advice to local populations to promote behavioural change and address health needs.
* Work to NICE guidelines on antenatal and postnatal mental health to identify, support and refer women at risk of postnatal depression.
* Maintain quality standards of care using audit and monitoring systems to evaluate the service.
* Identify vulnerable children in need of protection and refer to Social Care and Health following Birmingham Safeguarding Children Board policies; follow up on domestic violence notifications per Trust policy.
* Attend team around the family meetings, case conferences and participate in multi-agency child protection planning as agreed in roles and responsibilities.
* Plan, organise and run clinics/groups; arrange appointments for clinics and home visits in partnership with families.
* Develop and maintain effective working relationships with families, primary care team and other professionals in statutory and voluntary organisations.
* Ensure all equipment used by professionals is maintained and fit for purpose; order equipment and advise on safe use where appropriate.
* Promote and support immunisation programmes offered nationally to service users.
* Accurately enter data into the data collection system in line with policy; attend and participate in staff meetings and contribute to policy development within HV remit.
* Ensure written records meet NMC standards; write letters of referral and produce complex reports for court hearings relating to child protection; manage caseload including electronic recording and reporting.
* Provide professional advice on Health Visiting issues and promote understanding of health needs among other agencies; supervise, mentor and assess pre-registration student nurses and healthcare students where appropriate.
* Access safeguarding, caseload and clinical supervision to ensure a resilient workforce prioritising safe, compassionate care.
* Participate in Nurse prescribing – undertake full assessment of minor conditions and prescribe where appropriate; pursue continuing professional development in prescribing.
* Convey complex information to professionals and clients and work with others in difficult circumstances such as child protection, terminal illness or bereavement.
* Plan, organise and run clinics/groups.


Management and Leadership Responsibilities

1. Develop and empower all team members to perform to high standards and innovate.
2. Ensure supportive staff management arrangements are in place and carry out personal development reviews for direct reports, with annual PDRs and development plans.
3. Develop staff knowledge and skills to promote equality and diversity and address inequalities in employment and service delivery; include equality objectives in PDRs.
4. Develop a culture that supports Improving Working Lives and Investors in People standards and measures staff perceptions to drive improvements.


Qualifications

The post holder will be a first level registered nurse with a Health Visiting Degree/Diploma/Certificate and will work within the NMC Code of Professional Conduct.
