We looking for a dedicated Community Mental Health Support Worker/Assistant Practitioner to join our Mental Health Integrated Network Team (MINT). This is a key role focused on improving health, wellbeing, and care outcomes for individuals referred by their GPs, inpatient mental health units, crisis teams and single point of access.

* Maintain a live register of community resources
* Work closely with GPs, social care teams, and voluntary sector partners
* Respond appropriately to safeguarding or crisis concerns
* Help shape a system‑wide community asset map
Key Responsibilities
* Manage a caseload and offer one‑to‑one support
* Connect clients to relevant community services and activities
* Work collaboratively within the wider MINT team and external partners
* Monitor outcomes and maintain accurate records
* Participate in service improvement and professional development activities

* A strong communicator with a passion for mental health and community engagement
* Experience (or a strong interest) in health, social care, or community support work
* A proactive, solution‑focused approach and a commitment to person‑centred care
* An understanding of safeguarding, confidentiality, and working in multi‑agency environments
