West End Medical Centre is situated in Colwyn Bay, serving a list of 18,400 patients. The practice has recently merged two practices within the same building and focuses on delivering a social model of care. Our multidisciplinary team includes GPs, nurses, physiotherapists, and pharmacists, with plans to recruit advanced practice paramedics and physicians' associates.
Main duties of the job
The postholder will be expected to undertake all normal duties associated with GP work in primary care, including consultations via phone, video, email, home visits, providing advice, dealing with queries, paperwork, referrals, and timely responses.
Make professional, autonomous decisions regarding presenting problems, including undifferentiated and undiagnosed issues, whether self-referred or referred from other healthcare workers, adhering to best practice and evidence-based guidelines.
Screen patients for disease risk factors, early signs of illness, and promote public health interventions. Admit or discharge patients from the active case load and refer to other care providers as appropriate. Record clear, contemporaneous consultation notes to agreed standards and collect data for audit purposes.
