FarleighHospice is the leading specialist palliative care provider in mid Essex,providing specialist support to patients and their families in both thecommunity via our multidisciplinary locality care teams, and our inpatientunit.


Main duties of the job

We are now looking for a highly motivated Health Care Assistant to join our team on our Inpatient Unit.

Health Care Assistant Role - you would have experience and qualifications in the health care field. We welcome applications from people who have a passion for providing excellent care and support to our patient group. You will need a driving licence and access to a car for the community role

The 11-bed unit will provide holistic care for patients with both malignant and non-malignant life limiting illnesses and their families/carers. This will include excellent symptom control, psycho-social and spiritual support for patients who are admitted for symptom control, specialist palliative rehabilitation and end of life care; we are also aspiring to provide care for patients with complex nursing needs such as non-invasive ventilation and tracheostomy care.

Job responsibilities

MainPurpose

* As aHealth Care Assistant you will work within a multi-disciplinary team providingclinical care and support to hospice patients and their families
* Tomaintain effective communication skills to deliver emotive informationclearly, sensitively and unambiguously with staff, patients, families andprofessionals by face-to-face, telephone and/or electronic means.
* Towork collaboratively as part of the multi-disciplinary team (MDT) team sharingthe clinical caseload of patients/carers within the locality/in-patientunit for patients who have palliative care needs to ensure efficient and timelycare is delivered.
* Towork as part of the MDT, seeking advice from colleaguesand/or clinical leadership team when required, inorder to deliver excellent palliative and end of lifecare.
* Tosupport and participate in any streamlined locality/IPU processes and servicesby working to agreed quality improvement standards including clear outcomemeasures to produce efficient and highly performing clinical services.
* Tosupport the Hospice in building strong relationships with external partners andstakeholders and promote the good reputation of the Hospice e.g. local trusts,CCG, health and social care partners, businesses and voluntaryorganisations.
* Toensure that the service delivers an excellent standard ofcare to patients and their families.
* Toactively promote patient and family/carer involvement in care.
* Toact under the direction of the Locality Care Team Manager andprofessional colleagues and in accordance with written guidanceand procedures, undertake agreed clinical activities with designatedpatients.
* Tomonitor patients for any changes in symptoms andprovide non-pharmacological relief where possible, referringfor professional advice when required.
* Toprovide personal care, pressure area care, mouth care etc.as per the personalcare plan.
* Tobe flexible and creative in the care of the patient,andfamily. Involve and enable the family and friends to continue caring forthe patient and support them as appropriate.
* Toprovide emotional support and communicate effectively with patients and theirfamily/carers
* Towork unsupervised in the patients home as required.

Qualities/Attributes

* Honest, organised and reliable
* Maintains confidentiality of patients and their families
* Passionate about Hospice Care and enthusiasm for the vision of the organisation
* Team player who can work as part of our One Team ethos
* Flexible approach to work, with ability to work out of hours on occasions
* Committed to working in partnership with people their relatives and carers other organisations and agencies


Experience

* Previous experience of working in personal care.
* Experience in a palliative or end of life care setting.


Qualifications

* Good level of general education
* NVQ level 2


Skills/Ability/Knowledge

* Ability to empathise with the needs of the clients
* Ability to communicate effectively, both verbally and in writing, with a diverse client group.
* Ability to work alone and in a team
* Ability to work under pressureand in difficult/emotional situations
* Good understanding of English language spoken and written
* Good IT skills and the ability to input information onto computer systems

* Driving licence and car owner(community)
