We are looking to appoint an Occupational Therapist to join the Neighbourhood Rehabilitation Service (NRS) which includes Intermediate Care, Long Term Conditions and Crisis pathways, working across Barnsley Community.
The team provides rehabilitation to patients in their own home & residential care. The service provides specialist community therapy assessment & treatment to improve health & independence. In addition, the service offers rehabilitation to enable earlier discharge from hospital or to prevent hospital admission. You will work within a multi professional team & hold a varied caseload providing therapy input & high‑quality rehabilitation to people within their own homes & local care homes. You will assess, manage & provide treatment to individuals with complex and/or multiple needs, working closely with the MDT providing supervision/support to other staff. The team is committed to personal development & provides allocated time for CPD, training & wellbeing.
A vehicle to undertake travel is essential & the ability to work flexibly including evenings, weekends & bank holidays.

Main Duties of the Job
* Be responsible for the assessment & provision of a specialised programme of rehabilitation, meeting the specific needs & agreed goals of your patients to facilitate their rehabilitation & optimise recovery potential.
* Manage a clinical caseload with autonomy.
* Monitor and evaluate treatment to measure progress/outcomes to ensure effectiveness of interventions and service delivery.
* Develop programmes of care to support discharge.
* Contribute to clinical leadership/supervision facilitating a culture of learning/reflective practice.
* Have high level communication and excellent interpersonal skills to negotiate and establish collaborative partnerships with patients and care professionals.
* Ensure compliance with clinical governance by following Infection Control policy/procedures.
* Assist in service development and development projects as we strive to transform and modernise patient care.

Key Result Areas
* Prioritise and manage a significant caseload of patients within the Integrated Neighbourhood team.
* Assess for physical, cognitive and perceptual deficits in patients and to carry out assessment, treatment and use critical analysis to treat patients through goal setting, which is evaluated and adjusted throughout the patient's period of care.
* Assess the individual in the most appropriate environment and be responsible for the assessment, prescription and evaluation of specialised equipment, taking into account safe use/risk to patients and carers.
* Receive and communicate complex, sensitive and contentious information in a clear and effective manner with patients/family and carers. Overcome barriers such as hearing loss, fear, behavioural issues, language problems in order to maximise rehabilitation potential.
* Collect and effectively communicate information to relevant agencies, acting for and on behalf of the patient and carer, in order to obtain equipment or services and maximise collaborative working.
* Deputise for Band 7 in the management of the services for duties identified by them.
* Attend meetings, provide detailed written reports, risk assessments, discharge summaries and other relevant information and recommendations. Communicate effectively within the Multi‑disciplinary team, contributing to the decision making regarding discharge, planning and long term management.
* Participate in the induction and training of staff within multi‑professional, multi‑agency teams.
* Provide formal and informal supervision of junior grades of staff and rehabilitation assistants.
* Undertake the education of Occupational Therapy students whilst on clinical placement within the department.
* Comply with professional and trust policies and procedures, taking individual responsibility towards health and safety issues, undertaking risk assessments as appropriate.
* Maintain own continuing professional development in line with trust policy and professional guidelines, and undertake appropriate training and development to meet the needs of KSF/SDR.
* Participate in service develops in areas related to evidence based practice, service delivery and clinical governance.
* Contribute to the Occupational Therapy team, joint therapy team, multi‑professional team and older peoples service development.
* Maintain accurate and comprehensive patient treatment records in line with COT/BAOT guidelines.
* Provide supervision, training and development of Basic Grade Occupational Therapists, other health care professionals, support staff, and OT students, if present.

Person Specification
Qualifications
* Degree or equivalent in Occupational Therapy.
* Current HCPC Registration.
Training
* Evidence of Continuing Professional Development and training in relevant area.
Desirable
* Clinical educators qualification or achieved within 12 months.
Special Knowledge and Skills
* Ability to demonstrate specialised clinical reasoning and treatment skills.
* Good level of clinical reasoning, handling and treatment skills.
* Demonstrate effective communication skills.
* Demonstrate the ability to work as part of a team, in isolation and on own initiative.

Experience
* Demonstrable post‑graduate experience, in Older Peoples / Intermediate Care Services.
* Demonstrate experience of Evidence Based Practice and Risk Assessment.

* Demonstrate the ability to work under time constraints.
* Current driving licence and access to a car during the working day is essential (reasonable adjustments will be considered).
