Integrated Senior Support Worker (ISW) – Key Responsibilities
* Support people to wash and dress, promoting independence
* Encourage and motivate patients in managing their nutrition
* Medicines management from an administration chart under the supervision of an Integrated Senior Support Worker
* Work shifts from 8 am–10 pm, 7 days a week, with flexible full‑time and part‑time opportunities
* Independent travel between patients’ homes and the office base in a timely manner
The ISW role is an opportunity to provide outstanding care and support to patients in their own homes. The role involves working alongside Senior ISWs, Nurses and Therapists to optimise patient outcomes. ISWs provide personal care, medication support and aid with eating and drinking, while working towards further competencies to support the multidisciplinary team.
The Urgent Community Response team works alongside the integrated night service, providing cover overnight.
