An exciting opportunity has arisen for a committed and enthusiastic full time Community Mental Health Practitioner to work as part of the SBU HB Early Intervention in Psychosis Service. The team covers the whole of the ABMU Locality and works with individuals between the age of 14-25 years who are experiencing the early signs/symptoms of psychosis.
The post-holder will be responsible for providing community based interventions to service users and their families to reduce disruption and promote a good quality of life. The post offers the opportunity to develop your skills and experience whilst making a real difference to patient care.
The post-holder will need to demonstrate that they have good communication skills and it is essential that you possess the skills to work as part of a team. The post holder must have the confidence and personality to work autonomously, which is needed to undertake a lone worker role in the community.

Main duties of the job
Act as Care Coordinator, assess, implement and evaluate agreed CTPs for each individual on
caseload which has previously been agreed with service user/carer.
To work autonomously but under the indirect supervision of the Team Leader.
Act as a central point for coordinated data collection and compliance monitoring, working
closely with Locality Adult CMHTs, Tier 2/3 Specialist Community CAMHS, Voluntary Sector,
Adult Substance. Misuse Teams, Outreach & Recovery Teams, In patients and CRHTT’s
providing support and advice to clients with EIP
Engage with Service development initiatives driven by EIPN, NCAP standards and NICE guidelines

Knowledge & Experience
Essential criteria
1. Registered Nurse (Mental Health) or Social Worker, or Occupational Therapist. Up to date or equivalent registration.
2. Ability to demonstrate sound clinical knowledge and awareness of current evidence base relevant to the area of practice.
Desirable criteria
3. Possess advanced assessment and engagement skills. Understanding of current issues affecting service.
4. Evidence of effective partnership working.
Knowledge

5. Good time management skills.
6. Experience of working within a multi-disciplinary community mental health setting.
