Recovery Support Worker
Location: Fareham, Hampshire
Hours: 37.5 hours per week
Salary: £12.71 per hour
We have an exciting opportunity for a dedicated, passionate, and driven individual to join our caregiver team at Uplands House Care Home in Fareham. Uplands is a nurse-led service for people with mental health and physical health needs, located on the outskirts of Fareham.
Ideally, you will have experience in the care sector, specifically working with individuals who have mental and physical health needs. Familiarity with safeguarding and a clear understanding of personal development plans are also desirable.

Responsibilities:
* Provide care and support to residents, following their Support Plans to enhance their quality of life and wellbeing.
* Support residents with their physical and mental health needs, including personal care.
* Promote residents’ physical wellbeing through health checks, healthy lifestyle promotion, and social inclusion.
* Assist the Nursing Team in developing person-centred and inclusive Support Plans.
* Encourage residents’ involvement, independence, and choice in organizing their daily lives, in accordance with individual programs and policies.
* Support residents in integrating into and gaining new experiences within their community.
* Foster social acceptance through communal dining and social interactions.
* Build and maintain positive relationships with residents to support their recovery and create a therapeutic environment.
* Accompany residents on outings and shopping trips.
* Participate in meal preparation and serving, respecting residents’ preferences and dietary needs.
* Engage residents in daily activities aligned with the recovery program.
* Act as a co-key worker for residents, supporting their individual recovery plans.
* Support residents across all areas of their recovery plan, promoting a progressive care pathway.
* Demonstrate respect, acceptance, and tolerance, maintaining residents’ dignity and needs.
