Job Description:
Liquid Personnel is currently recruiting for experienced and qualified Social Worker to join a busy Hospital Discharge Team based in Kingston.
What will your responsibilities be?
Ensure compliance with conduct codes and legislation, manage complex caseloads, and assess customer needs. Supervise staff, maintain qualifications, and develop partnerships. Promote team practices, ensure safety, handle escalations, and stay updated on service developments. Conduct risk assessments and reviews. Communicate effectively, maintain relationships, share knowledge, and contribute to team performance. Commit to development, act on feedback, and ensure high-quality service. Support managers, maintain confidentiality, work off-site, and ensure cost-effective service. Challenge practices, adhere to standards, and undertake additional duties. Specialize in case management, safeguarding, and occupational therapy. Conduct assessments, develop support plans, ensure timely interventions, and handle safeguarding concerns. Recommend equipment, manage adaptations, and support rehousing assessments. Progression depends on performance, development, and service standards. Demonstrate leadership, supervise staff, ensure quality, support team leaders, and contribute to improvement.

Qualifications and Experiences:
To be successful in this role, you must have experience within adult social care and completing Mental Capacity Assessments, as well as setting up packages of care and support planning. It is essential to identify areas of need and/or risk of harm and ensure appropriate safeguarding practices and measures are observed and in place at all times. Additionally, you need Social Work England registration, eligibility to work in the UK, a full UK driving licence, post-qualified experience, and a degree level or equivalent qualification in Social Work.
