To facilitate early discharge and reduce re admission to hospital.
Carrying out home visits to provide education and support to families to enable them to care for their child safely at home.
Run nurse led clinics - carrying out nursing activities closer to home.
Co-ordinating the children's Community Nursing Team.

JOB SUMMARY
This post is designed to facilitate early discharge and prevent avoidable readmission to
hospital for children with either acute or chronic health care needs, through the provision
of specialist support from an autonomous practitioner to their family or carers.
The seamless transition from hospital to home will be achieved through multi –
Professional / multi – agency working, across all health care settings.
To protect, safe-guard and promote the well-being of children in accordance with ‘The
Children Act, 1989’ and ‘Working Together Under The Children Act, 1989’.
RESPONSIBILITIES
CLINICAL:
1. Undertake assessments of care needs within the community. Act in partnership with the child and family when assessing a child’s needs and planning, implementing and evaluating care in accordance with the children’s community nursing service philosophy of care.
2. Discuss children on their case-load with designated senior team members
3. Reassess care needs and care outcomes through ongoing discussion and negotiation with families / carers.
4. Provide specialist support to families / carers to enable them to become confident in their ability to meet their child’s health care needs at home or in a clinic setting.
5. Act as an expert resource to the child and family in their own home or in the clinic setting.
6. Share expert knowledge within the profession and with core professionals, and provide specialist advice within the domiciliary and clinical area.
7. Recognize opportunities to facilitate and develop health promoting activities in the home or in a clinic setting.
8. Identify the need for and utilize counseling skills for both the child and their family as appropriate.
9. Maintain precise documentation relating to patient care according to the NMC standards for record keeping and provide such statistics and information as required by management.
10. Act as a role model and a resource / advisor to other professionals within the trust and the primary health care team.
11. Maintain clinical expertise and develop research based practice skills, within the scope of professional practice. Accessing specific / relevant training to enhance clinical skills where appropriate.
12. Be conversant with the procedures and information within the Lancashire Child Protection Committee Handbook.
PROFESSIONAL:
1. Maintain active status on the NMC register
2. Act always in accordance with NMC Code of Conduct and guiding documents.
3. Maintain up to date skills, knowledge and professional portfolio and be aware of current professional issues.
4. Evaluates own practice against the KSF Outline for the post and meets the requirements of the KSF Review Process.
5. Maintain legible, accurate records and documentation and monitor the effectiveness of the established communication / reporting system, in compliance with the NMC Code of Conduct.
6. Follow agreed policies and procedures of the trust and promote the Quality Assurance Initiative.
7. Raise the profile of the service by good communication and public relations.
8. Attend regular team meetings in accordance with mandatory requirements.
MANAGEMENT:
1. Be responsible for a case load and assess, plan and evaluate its management.
2. Create and maintain effective communication channels with General Practitioners, Health Visitors, District Nurses, School nurses, Regional Nurse Specialists, Hospital Professionals, Patients and family and liaise with other agencies as appropriate.
3. Be familiar with statutory and voluntary support services and agencies available to families with sick children.
4. Support and participate in quality assurance initiatives working towards achieving the highest standard of care.
5. Implement and maintain the directorate and trust’s quality initiatives and assurance programmes relevant to the clinical area.
6. Participate in resource management and the management of available resources.
7. Monitor and maintain equipment used by the CCN service
8. Be involved in the orientation, induction and preceptorship of junior staff
9. Foster good interpersonal relationships within the team in order to achieve a good working environment for the team members.
10. Ensure patient confidentiality is maintained at all times.
11. Advise clients on the complaints procedure and when necessary take prompt and appropriate action on receipt of complaints and participate in official investigations when necessary.
12. Identify areas for improvement/development of the service and take appropriate action by providing relevant information by incorporating user views.
13. Seek support and advice in matters beyond own sphere of knowledge and experience.
TEACHING:
1. Assist in ensuring that the learning environment complies with the NMC and UCLAN requirements.
2. Participate in supporting, mentoring and the assessment of learners on the Community placement.
3. Complies with the trust’s training strategy.
4. Complies with the directorates and trust’s business plans.
HEALTH AND SAFETY:
1. Complies with safety instructions and policies laid down.
2. Uses, in a proper and safe manner the equipment and facilities provided and report as practical any hazards and defects to the relevant department.
3. Refrains from the willful misuse or interference with anything provided in the interest of health and safety and any action which might endanger yourself and others.
4. Reports as soon as practical any accidents on untoward occurrences and to ensure that incident forms are completed.
5. Submit written reports of any accidents, complaints or grievances occurring to any patient, visitor or member of staff and take part in investigations as required.
STANDARDS OF CONDUCT:
1. Ensure that personal actions and conduct comply with the Trust Policies and
Procedures e.g. Health and Safety, Tobacco Control Policy, Equal Opportunities,
Confidentiality, the Data Protection Act (1998) and Moving and Handling

Post holders are expected to work flexibly within their pay band. They should only be expected to carry our activities for which they are competent.
