Overview
Mersey and West Lancashire NHS Foundation Trust is seeking a highly motivated and experienced Band 8a Clinical Pharmacist to join our dedicated team at Southport and Formby District General Hospital. This role offers a fantastic opportunity for an enthusiastic pharmacist to play a key role in delivering high‑quality clinical pharmacy services and optimising medication management for inpatients.
Responsibilities
* Taking drug histories on admission and completing medicines reconciliation for inpatients.
* Screening newly admitted patients for medicines optimisation issues and developing individualised care plans.
* Conducting medication reviews to ensure safe, evidence‑based, and cost‑effective prescribing.
* Monitoring and adjusting patient medication based on therapeutic drug monitoring results and response to treatment.
* Counselling patients on their medication, ensuring they understand their drug therapy and any potential side effects.
* Providing professional and legal clinical supervision in the dispensary, ensuring best practices in dispensing and medicines management.
Qualifications
Essential Criteria
* Master's degree in pharmacy (MPharm) or equivalent qualification.
* Registration with the General Pharmaceutical Council (GPhC).
* Evidence of ongoing Continuous Professional Development (CPD)/Revalidation in line with professional requirements.
* Previous relevant hospital experience in a clinical setting.
* Strong foundation in Clinical Pharmacy and Medicines Optimisation.
* Excellent verbal and written communication skills to liaise effectively with multidisciplinary teams.
* Strong time‑management, decision‑making, and organisational skills.
* Computer literacy and experience using clinical systems and electronic prescribing tools.
Desirable Criteria
* Postgraduate qualification in Clinical Pharmacy or independent prescribing qualification.
* Experience in a specialist clinical area such as acute medicine, critical care, or cardiology.
* Previous experience supervising and mentoring junior pharmacists or pharmacy technicians.
* Familiarity with NHS policies and procedures in medicines management.
