The purpose of the Lung Health Screening Programme is to enable the early detection of lung cancer diagnosed at Stage 1 and 2 to improve survival rates for those patients diagnosed with cancer. The Lung Cancer Screening Programme targets those most at risk of lung cancer. The team offers Lung Health Checks to patients between the ages of 55-75 with a history of smoking. This role will also work with the Lung Cancer team to provide support and advice to people in accessing appropriate information by signposting to a variety of resources. Promote and educate towards a self-management approach, where appropriate. Contribute to Holistic Needs Assessments and the development of an individual Care Plan in line with practitioner guidance, protocols, and procedures.
* Carrying out Lung Screening Health Check assessments with patients
* Carrying out baseline observations
* Use of IT systems such as Soliton, Infoflex, Sunrise, and others as required
* Working closely with the clinical team to support patient flow and ensure a positive patient experience
* Booking of appointments
* Contributing to Holistic Needs Assessments and developing individual Care Plans
* Referring and signposting patients to the clinical team for complex decisions
* Supporting oversight of the PTL to meet 28-day (FDS) and 62-day standards for cancer, escalating issues to the Lead Lung CNS as needed
* Risk assessing patients individually, referring to other services appropriately, and triaging within competencies
* Communicating effectively with MDTs and clinical teams to support coordinated, patient-centered care
* Supporting and advising people in accessing information, signposting to resources, and promoting self-management where appropriate
* Liaising with all team members, patients, and carers to ensure high-quality care
