Role Overview
Intensive Community Support Dementia team (ICSD) is seeking a Clinical Team Leader to oversee community-based care for patients with dementia and challenging behaviours. The role involves leading a small team, coordinating care and risk management, and ensuring high quality interventions in both residential and home settings.
Responsibilities and Duties
* Conduct clinical assessments, nurse reviews and manage a small group of patients.
* Administer triage and accept referrals from CMHTs, social care and other agencies.
* Lead shift management, ensuring smooth team operation and effective resource utilisation.
* Develop and implement service changes in line with the National Dementia Strategy and the Recovery Agenda.
* Prepare comprehensive care formulation and risk management plans.
* Complete the Prevention and Management of Violence and Aggression course; completion is a condition of employment.
* Communicate clinical decisions and provide support to carers and patients throughout the care pathway.
Qualifications and Experience
* RMN or OT with community and dementia experience.
* Strong assessment, risk management and care formulation skills.
* Excellent communication, clinical decision‑making and leadership abilities.
* Knowledge of MHA, MCA and safeguarding legislation.
* Valid driver’s licence and availability to use a driver’s vehicle; pool cars are available.
* Experience in a 7‑day, 07:30‑20:00 community service is desirable.
Working Hours and Conditions
This is a part‑time role, minimum 30 hours per week, based at Forston Clinic, Dorchester. The service operates on a rota basis 07:30‑15:30 / 12:00‑20:00, covering alternate weekends at enhanced pay. A hybrid approach and remote working opportunities are welcomed.
