We are recruiting for a Band 3 Coordinator/Administrative Assistant to join our Interim Care Team at University Hospitals Dorset (UHD).
Our well-established and highly regarded team provides an early supported discharge service across two sites: Bournemouth Hospital and Christchurch Hospital. We support patients who are ready to leave the acute hospital setting but require additional help to regain and optimise their independence at home.
The service is therapy- and nursing-led, with a multidisciplinary team that includes Coordinators, Community Therapy Assistants, Occupational Therapists, and Nurses. Currently, the team comprises one Band 4 Coordinator and two Band 3 Coordinators.
The post holder will report to the Clinical Leads for both the Interim Care Team and the Discharge Team. The role involves coordinating discharges from UHD hospitals and supporting the delivery of a high-quality service. Key responsibilities include managing administrative tasks such as data collection, handling telephone and email enquiries, and ensuring the efficient day-to-day running of the service. The post holder will provide a professional, responsive, and supportive service to colleagues, partner organisations, patients, and their families.
Some weekend working may be required.

Key responsibilities include:
1. To coordinate discharge referrals from UHD, ensuring all required information is provided and procedures are followed.
2. To handle administrative tasks relevant to the interim care team.
3. To work closely with hospital therapists, social work teams, and discharge keyworkers to deliver high-quality, efficient discharges.
4. To coordinate the therapy assistant’s timetable, ensuring adequate time for care visits, travel, and breaks.
5. To produce accurate reports within strict deadlines.
