How youll make a difference

As a Discharge to Assess Coordinator, you will help coordinate the discharge process for service users who need extra support when leaving hospital, ensuring they return to the most appropriate setting for a further assessment of their long-term care and support needs.

Based at the Transfer of Care Hub in Southmead Hospital, you will work with different partners to make sure service users are on the right pathway to move safely from hospital to home care, ensuring discharge happens smoothly and quickly, and everyone is informed about the discharge progress.


What you will be doing
* It will be key for you to maintain and update information systems and databases, enabling service transactions to be completed accurately.
* You will coordinate the safe and timely discharge of service users from hospital to their appropriate pathway.
* Working closely with social workers, hospital staff and care providers, you will ensure they receive appropriate guidance about policies and procedures, whilst also building and maintaining effective working relationships.
* It will be your responsibility to respond promptly and appropriately to all queries and requests, escalating any issues that cant be resolved to more senior members of the team.
* Occasionally, you may be involved in carrying out research, collating data, presenting information and documenting findings.
* You will provide a wide range of administrative tasks, including maintaining multiple spreadsheets to accurately record and monitor the work of the hospital team.

What we need from you
* We require you to be educated to a minimum of 3 grade C GCSE’s (or equivalent) including English and maths.
* You must have a good understanding of working in the community in a social care or health setting, with the ability to understand individual needs.
* It is essential that you have experience of collating and distributing a range of information relating to service users.
* You will have a good knowledge of Microsoft Excel and IT skills to use a client data system.
* Given the nature of this role, you will have some experience in a relevant area of statutory or voluntary sector work in the health or social care field.

What you need to know
* Your salary is pro-rata to hours worked.
* The working pattern is 9am to 5pm, 3 days per week. We will require one of your working days to be a Friday.

* This role is hybrid, you will mainly be office based at Southmead Hospital, with opportunities to work from home.

Interviews are likely to take place week commencing 13th April 2026.
