You will work as part of the wider team of coordinators within our Fair Havens Community Team to ensure that our processes and HRAS referrals are streamlined with robust internal pathways. You will undertake administrative duties as required.
1. To be responsible for data collection for all aspects of placements utilizing SystmOne records and computerised databases.
2. To work closely with clinical staff within the Core teams to deliver the above service and escalate clinical concerns and sourcing difficulties to the Rapid Access team and the Head of Adult Services/Community Lead.
3. To support the Rapid Assessment and Discharge Team with administrative support as required.
4. Through telephone, virtual, and face-to-face discussions, provide information to patients with palliative and end-of-life care needs, support their families and carers, liaise with clinical professionals, and refer on as necessary to appropriate health and social care partners.
5. To support the Hospice in building strong relationships with external partners and stakeholders, promoting the good reputation of the Hospice, including local trusts, ICB, health and social care partners, businesses, and voluntary organisations.
6. To role model the values of Havens Hospices and support others within the organization by recognizing positive behaviors, challenging poor behavior, and encouraging change.
7. To be visible and credible, listening and responding to staff and patient/family issues and insights.
8. To communicate effectively across the organization to promote teamwork and collaboration.
9. To work as part of the Rapid Access to Care Team and provide cover during absences.
10. To support the team in producing personal care reports and activity data.
11. To demonstrate negotiation skills in dealing with challenging situations, managing patient/carer expectations, and reporting conflicts.
12. To manage personal performance, identify skills and competencies, and develop personal development plans.
13. To support the Clinical Quality Team in collecting and collating performance data for service improvement.
14. To collaborate with the Community Lead Nurse to streamline processes using quality improvement methodologies and clinical outcome measures.
15. To ensure contracts are prepared and signed following SOPs.
16. To negotiate best value for placements.
17. To escalate clinical or operational issues promptly.
18. To input administrative information accurately.
19. To ensure personal care data entry and collection meet quality standards.
20. To train relevant staff in procurement and data entry requirements.
21. To work with the Service Lead to establish and implement systems supporting the service.
22. To communicate clearly and sensitively with staff, patients, families, and professionals via various communication methods.
We are seeking a full-time Care Co-ordinator to join our team at Fair Havens Hospice, located near Priory Park in Southend on Sea. This role supports a 7-day service, requiring availability across all days, including weekends and bank holidays.
