At Making Space, we support individuals and communities to live well, and we are now looking for a Carer Assessment Worker to join our Cheshire East Carers Service.
The role
This is a community-based, part-time position (30 hours per week) covering Macclesfield and surrounding areas, or the south of Cheshire East. You will work closely with unpaid carers, offering one-to-one support, guidance and assessments within their homes and local community settings.
A full UK driving licence and access to your own vehicle are essential for this role.
Key Responsibilities
* Carry out carer assessments within home and community environments
* Provide clear advice, guidance and information tailored to individual needs
* Support carers to develop practical coping strategies
* Offer emotional support and signposting to relevant services
* Share information relating to mental health and available local support
* Maintain accurate records and work in line with safeguarding practices
