This is a rare and exciting opportunity for you to come and join our friendly team. We are a multi agency team which provide health and wellbeing education and promotion to adults and older people in their own homes, over the telephone and via other routes i.e. group sessions and community events. We also help deliver the NHS Health Checks program in the community and at outreach venues across West Sussex. This is a great role for a community nurse with the enthusiasm for Health Promotion. It\'s a job that comes with flexibility and the scope for you to bring in your ideas on how to reduce health inequalities across West Sussex. The Nurse Advisor role is varied, focusing on home/telephone assessments, NHS Health Checks, and outreach. The Advisor completes holistic assessments of patients’ needs either by phone or during home visits. After appropriate training, they deliver NHS Health Checks in community settings, supporting individuals in accessing health and wellbeing services through behaviour change—part of services commissioned by West Sussex County Council’s Public Health Department.
Responsibilities
* Carry out holistic assessments of patients’ needs by phone or during home visits.
* Deliver NHS Health Checks in community settings after appropriate training.
* Support individuals in accessing health and wellbeing services through behaviour change.
* Work in collaboration with PAT colleagues, primary care teams, social services, and the voluntary sector to ensure integrated service delivery and avoid duplication.
* Help cover the Duty Line and maintain a service directory for accurate signposting.
* Ensure resources target populations with the greatest need, guided by Public Health data and the multi-agency PAT Board.
* Occasionally supervise student nurses during community placements (not a managerial role).
Qualifications and Experience
* Registered as a nurse with enthusiasm for Health Promotion (community nursing experience preferred).
* Ability to conduct home/telephone assessments and participate in NHS Health Checks in community settings.
* Strong collaboration skills to work with multi-agency partners and signposting resources.
* Flexibility in working arrangements and willingness to travel within West Sussex.
* Capacity to support and supervise student nurses during placements when required.
