Community Specialist (Community Links Practitioner)
The ALLIANCE is recruiting three new Community Links Practitioners to join our team based in Glasgow. Positions are full‑time or part‑time and will support GP practices in socio‑economically deprived areas.
* Full‑time maternity cover – 35 hours per week, fixed term until December 2026.
* Part‑time (17.5 hours per week) – fixed term until 31 March 2026, subject to continued funding.
* Part‑time (21 hours per week) – fixed term until 31 March 2026, subject to continued funding.
The Community Links Practitioner will provide a person‑centred service that is responsive to the needs and interests of GP practice populations. Responsibilities include:
* Support patients to identify issues affecting their ability to live well and assist them in decision‑making processes.
* Use service‑improvement methodology to help primary care teams adopt a links approach.
* Facilitate connections between community resources (e.g. social/lunch clubs, befriending organisations, employment agencies) and patients.
* Build relationships with statutory organisations, health services and voluntary organisations such as the Citizens Advice Bureau, Money Advice Service, NHS services and charities.
* Support people to live well by strengthening connections between community resources and primary care in deprived areas.
