Band 6 Community Mental Health Nurse role at Sheffield Health Partnership University NHS Foundation Trust

An exciting opportunity has arisen to work within our Older Adult Community Mental Health Teams based at Edmund Road. The Older Adult Community Mental Health Team supports people aged 65 and over who are experiencing mental health difficulties. We support people who are experiencing problems such as psychosis, mood disturbance and memory problems, who have related behavioural or psychological symptoms. The team covers the entire city and provides support in local communities, including home visits or visits to care homes.

Older Adult Community Services are currently engaged in a process of transformation. You should be able to demonstrate your passion for working with this client group and have the ability to assess, co-ordinate treatment and support clients and carers. You should possess effective communication skills and have the ability to work under your own initiative and as part of the wider Multidisciplinary team. A professional qualification in Mental Health Nursing is essential.

To work across a variety of community settings as part of a multi-disciplinary team. Providing a nursing assessment and identification of clients' mental health needs. Formulating, implementing and evaluating nursing care plans that reflect the needs of the clients/carers over a 24-hour period. Manage own caseload with responsibility for the nursing care of clients until discharge. Administering depot injections to clients and monitoring the effects of prescribed medication. To act as co-ordinator / key worker. Delivering a range of therapeutic interventions with clients and carers in accordance with evidence-based practice and Trust policies. Reacting to and managing a range of complex mental and physical health crises. Offering support and advice to families and carers. Managing and maintaining a caseload with appropriate levels of supervision. To provide direct supervision to other staff and contribute to the PDR system. Liaising with, advising and educating other professionals, agencies and bodies involved in the care of older people with mental health needs. To act as identified practice supervisor and practice assessor for nursing students on allocated placements; also to act as Preceptor to nurses completing their preceptorship. A professional qualification in Mental Health Nursing is essential.

Job purpose and responsibilities include valuing and being responsive to the individual rights, needs and opinions of older people using our services, and performing all activities of the Community Mental Health Nurse. Be responsible for ongoing assessment, planning, implementation and evaluation of nursing care to an agreed standard. Undertake initial mental and physical health assessments. Participate in monitoring the standard of care delivery by junior staff and the education of colleagues, learners and junior staff. Maintain and promote a client-centred approach in all aspects of service delivery to older people and their carers.

General requirements The post holder is expected to be familiar with relevant policies and legislation, and to work within this framework, including NMC guidelines and trust policies. Through supervision and PDR, the post holder will construct a personal development plan to achieve key result areas.


Responsibilities

Clinical: To work across a variety of community settings as part of a multi-disciplinary team. Provide nursing assessment and identification of referred clients' mental health needs in home or community settings. Formulate, implement and evaluate nursing care plans reflecting client/carer needs over a 24-hour period. Manage own caseload with responsibility for care until discharge; regularly review caseload with clinical supervisor. Administer depot injections and monitor medication effects. Maintain accurate care records according to professional and Trust standards. Act as co-ordinator in accordance with the Care Programme Approach. Provide comprehensive written mental health specialist nursing reports as required. Deliver a range of therapeutic interventions with clients and carers in line with evidence-based practice and Trust policies. Manage crises; escort clients to other health and social care settings when appropriate. Offer support and advice to families and carers and provide advocacy where appropriate. Identify and manage risks in various settings. Leadership/Supervisory: Provide supervision to staff and contribute to the PDR system. Liaise with and educate other professionals and agencies involved in care of older people with mental health needs. Act as practice supervisor/assessor for nursing students and as a Preceptor for preceptees. Receive clinical supervision regularly. Service development: Promote high standards of professional practice, enable access to training/education, and share skills with colleagues and learners. Act as a positive role model and provide education and health promotion to clients and carers.
