An exciting and rare opportunity has arisen to join our Adult Social Care department as a Social Worker in the Preparing for Adulthood Team.

The Opportunity
We are looking for an enthusiastic and motivated individual with the relevant experience in Learning Disabilities and Neurodiversity to join our dedicated Preparing for Adulthood Social Care Team. You will undertake Assessments, Reviews, Safeguarding and arrange the delivery of social care services, under the Care Act 2014, to the Citizens of Derby who are transitioning from Childrens to Adult services. These citizens may have Learning Disabilities, Physical Health Concerns, EHC Plans and/or Mental ill-health. We aim to support those individuals to develop their skills and abilities to live a life of their choosing and fulfil their potential. You will be liaising with key stakeholders, families and commissioning colleagues to build the right support around individuals with complex needs.

About You
Applicants must have experience of working in teams supporting those with Learning Disability/Mental Health conditions/Vulnerable Adults/Physical Health disabilities in Statutory Services. You must be able to support colleagues to develop their knowledge, skills and personal resilience in a challenging environment through peer support and take responsibility for your own ongoing learning and improvement through reflection and quality assurance. You will have experience of promoting social justice and self-determination. A Social Work qualification is essential.


The successful candidate will be part of a team striving to make improvements in the way we deliver services, working closely with customers, carers and partner agencies across the Public, Private and Independent Sectors for purposes of coproducing service improvement.
