If so, this may be the job for you! Due to the existing post holder relocating we have a vacancy for a band 4 Assistant Practitioner to join our falls service.
This role is varied and interesting, no two days will be the same, delivering a mix of clinic and home visiting sessions, with the team providing a range of specialist interventions including aquatic therapy and a range of exercise intervention.
Main duties of the job
Working as part of the specialist falls service delivering clinical and therapeutic care, within set protocols and within scope of Assistant Practitioner remit.
To undertake and complete appropriate training and competencies relating to a falls specific role in order to carry out the Assistant Practitioner role.
Monitoring long term conditions, care, and treatment plans as per policy, protocol or guidelines within a care episode, escalating to registered staff for support and guidance when required.
Prioritising own workload, being able to work autonomously with registered HCP oversight.
Demonstrate procedures to and supervises non-registered staff and contribute to the learning environment.
Act as a role model by upholding and implementing good practice in the workplace, always ensuring the highest standards of evidence‑based care.
To maintain accurate, timely written and computerised documentation in line with professional guidelines and legal requirements.
To evaluate and risk assess all aspects of community work and to minimise the risk to self, patients, and carers whilst delivering care.

Qualifications and Training
* Successful completion of relevant Foundation Degree modules / Diploma in Clinical Health and Social Care / diploma level 3 or equivalent
