We are expanding our multidisciplinary team and we're looking for a motivated, patient‑centred Pharmacist to join our friendly, forward‑thinking coastal practice. You'll play a key role in medicines optimisation, long‑term condition management, and supporting safe, effective prescribing across our three sites. This role is ideal for a pharmacist who thrives in a collaborative environment, enjoys problem‑solving, and wants to make a meaningful impact on patient care within a supportive NHS primary care setting.
Responsibilities
* Medication reviews – structured medication reviews (SMRs), polypharmacy reviews, and medicines reconciliation
* Long‑term condition support – contributing to management of hypertension, asthma, COPD, diabetes and more
* Clinical support – assisting GPs with complex prescribing queries and safety monitoring
* Quality improvement – contributing to audits, QOF, IIF and medicines safety initiatives
* Patient education – supporting patients to understand and manage their medicines
* Collaborative working – working closely with GPs, nurses, HCAs and other multi‑agency colleagues
Qualifications
* GPhC‑registered Pharmacist
* Experience in primary care, community, hospital or PCN settings
* Strong clinical reasoning and communication skills
* Proactive, organised, supportive team player
* Independent Prescriber qualification (or willingness to work towards it)
Skegness Full‑time / Part‑time – Salary dependent on experience.
