Job overview

As a registered nurse experienced in CHC, you will join a busy team of nurses who play a pivotal role in the CHC process.

Knowledge and understanding of the current national and local policy/Framework Guidance relating to NHS Continuing Healthcare and NHS Funded Nursing Care is essential.

Main duties of the job

Under the guidance and support of the Clinical Lead, the Nurse Assessor will assess individuals for NHS Continuing Healthcare, in line with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing care, revised July 2022. The Nurse Assessor will promote best practice and provide professional advice to Nursing Homes and other providers, promoting good relationships and liaising with colleagues from other disciplines both internally and externally to the Integrated Care Board (ICB). Assessments are completed face to face.

The post holder will be required to follow due process for determining eligibility for NHS CHC for live and retrospective cases, as well as appeals and dispute cases.

As part of the review process, the Nurse Assessor will review the eligibility for the NHS CHC, as well as the care plan, to ensure that all care needs are being met safely and effectively. You will monitor and influence the quality of care that is provided to patients; drawing on previous skills and knowledge to improve the patient experience of care. You will provide professional advice to patients, carers and multi-agency colleagues, including signposting to appropriate services and resources.

The Nurse Assessor will contribute to the safeguarding adults from abuse process (SAFA) by undertaking reviews of individuals who have been identified as at risk of harm; as part of this process you will have a role in the monitoring and reviewing of placements/packages of care.

Working closely with partner organisations, the ICB's staff design, plan and commission health services for more than half a million people and monitor the quality and effectiveness of those services.The ICB currently has vacancies for Nurse Assessors within the Continuing Healthcare (CHC) team.

In addition to the basic salary, these posts attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum and maximum payment per annum (pro rata where applicable) in line with Agenda for Change.

The posts can be based at our sites at Welwyn Garden City, Hemel Hempstead and Harlow. The post-holder will need to be able to travel between sites.

Qualifications and Knowledge

* Current 1st Level Registered Nurse/NMC Registration
* Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
* Knowledge and understanding of current national and local policy/Framework Guidance relating to NHS Continuing Healthcare and NHS Funded Nursing Care
* Knowledge and experience of working with Safeguarding issues and Policy guidance
* Experience of working with patients with multiple /complex needs


Experience

* Experience of working across organisations, local authority, other NHS Trusts, voluntary and independent sectors
* Knowledge of administrative procedures, project management or information analysis
* Problem solving skills and ability to respond to sudden unexpected demands


Communication

* Clear communicator with excellent writing, report writing and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences
* Evidence of success in efficient and effective project and programme management
* Skills for communication on complex matters and difficult situations, requiring persuasion and influence.
* Skills for nurturing key relationships and maintaining networks.


Analytical

* Ability to analyse and interpret information, pre-empt and evaluate issues, and recommend and appropriate course of action to address the issues
