We need people who share our excitement, embrace different ways of working and are able to work flexibly across organisational boundaries supporting the NHS Long Term Plan, providing rehabilitation and safe discharge home. The post holder will work as part of the multi‑disciplinary team, providing rehabilitation and planning discharges. They will work in line with the agreed strategic objectives of Northern Care Alliance.
