An exciting opportunity has arisen for an experienced Bank Community Mental Health Nurse/Social Worker Band 6 to work within the South Beds Leighton Buzzard CMHT.
We are looking to recruit someone who is highly motivated, enthusiastic, and passionate about applying the Biopsychosocial intervention/recovery model within a community setting. This role is suitable for someone with a strong interest in Community Mental Health Services.
This position will be based at Crombie House, Leighton Buzzard. You will work with a multidisciplinary team including CPNs, Doctors, Psychologists, Support Workers, and Occupational Therapists.
As part of the NHS Long Term Plan and the vision for Community Mental Health, you will play a key role in achieving the aspirations of service users and carers, focusing on people's assets and strengths, and considering holistic, creative solutions to support individuals.
You will work in partnership with service users and carers, identifying and utilizing their strengths, and collaborating with other agencies and voluntary groups to ensure good outcomes that maintain independence as much as possible. A good working knowledge of The Care Act, Mental Health Act, Mental Capacity Act/DOLS, and other social care legislation is required.
Responsibilities include:
1. Managing an identified community caseload, ensuring high standards of professional conduct and compliance with best practices.
2. Delivering statutory social care duties under relevant legislation in a complex, multi-disciplinary environment.
3. Accessing appropriate care and support packages for people with mental health needs and/or their carers, following local authority procedures.
4. Promoting a service user-focused approach.
5. Participating in audits and surveys relevant to the team.
6. Contributing to the development of new policies and procedures.
7. Ensuring services are culturally competent, respecting equal opportunities and human rights.
8. Establishing and maintaining good relationships with service users, carers, and voluntary sector colleagues.
9. Providing evidence-based practice that complies with legislation, policies, procedures, and quality standards.
