What You'll Do
As our Care Coordinator, you'll be the heartbeat of our new Chelmsford service, ensuring smooth operations and exceptional care for our clients.
Your role will include:
* Coordinating rotas and visit schedules to deliver consistent, high-quality care.
* Building strong relationships with clients, families, and healthcare professionals.
* Monitoring care quality and ensuring CQC compliance from day one.
* Supporting and mentoring our care team to help them thrive and grow.
* Sharing participation in the on-call rota (1 in 4 weeks) to keep services running seamlessly.
What You'll Bring
We're looking for someone with a mix of compassion and organisational skills, ready to step into a leadership role.
You'll need:
* 1-2 years of experience in a care setting, ideally in domiciliary/community care.
* At least 1 year of care coordination experience.
* A Level 3 Health & Social Care qualification (or willingness to achieve it).
* Excellent communication and problem-solving skills.
* Confidence using digital systems for scheduling and record keeping.
* A full UK driving licence and access to a reliable vehicle.
* A proactive, empathetic approach with the ability to stay calm under pressure.
Job Type: Full-time
Pay: £28,000.00-£30,000.00 per year
