The apprentice will work as part of the school nursing service in the community. He/she is responsible for the assessment of patient care needs under the supervision, guidance and support of a Community Practice Supervisor and Assessor.
You will be responsible for the delivery of the Health Child Programme. You will identify additional needs of children and their families implementing packages of care under supervision in order to improve health outcomes.
You will co-ordinate the case load ensuring effective communication is maintained at all times across all stakeholders. Your time will be spent equally distributed between your practice placement and the university.
You will support a culture of learning and reflective practice and contribute to the Trusts Clinical Governance Strategy and Public Health agenda, whilst ensuring that you meet the learning outcomes set by the educational establishment.
The apprentice will utilise the theory learnt at university to consolidate learning, learn new skills and deliver evidence-based care delivery on practice days within the school nursing team.
Whilst On Placement The Apprentice Will
* Work along-side their practice assessor, practice supervisor and other health/ multi-agency professionals to learn the role of a specialist community public health nurse- (school nursing)
* With support will attend multi-agency meetings in order to work cohesively with health, education, police, children’s social care, local authority and other external agencies to optimise and advocate for the safety, health and well-being of children and families on the school nursing caseload
* With support will support the delivery of health promotion and prevention programmes inline with the healthy child programme
* With support will learn essential safeguarding skills. With support will learn Including health assessments, report writing and sharing of relevant health information in line with GDPR
* Will have the opportunity to learn leadership skills; carry out audits, attend service planning meetings, help to shape policies, guidelines, and evidence- based interventions to promote positive change and enhance quality of care delivery.
* There will be a consolidation period where the apprentice will consolidate their learning and practice autonomously with regular supervision from their practice assessor and practice supervisor
