Specialty Doctor Care of the Elderly - BAH, DMH

The post holder will be joining a dynamic andfriendly medical team to develop and embed high quality care for older patientsin a number of different clinical settings alongside the existing specialitydoctor and consultant team. We have established acute front of house frailtyteam, acute complex frailty wards, specialist movement disorder service,ortho-geriatrics, surgical liaison, general geriatric clinics and falls/syncopeand Tilt clinic. As a result we are able to explore a candidates interest andtry to tailor a post to their development, appreciating that a doctor workingin a team within their area of interest is a valuable asset in driving forwardservice development and high quality. Therefore, applicants must have suitabletraining and an interest or wishing to gain experience in Geriatric Medicine

The post holder will haveappropriate training as outlined in the job specification and a well -developedmedical knowledge, allowing the ability and confidence to function decisivelyand autonomously. Candidates should be hard-working, dependable, collegial, andprofessional in keeping with GMC good medical practice.

Youmust be fully registered with a GMC license to practice.


Main duties of the job

The successful candidate will join a team providing highquality care for medical patients within the elderly care / frailty service.

Supportand develop the acute front of house frailty assessment unit, acute complexfrailty unit, frailty specialist units and rehabilitation wards.

Supportoutreach services to surgical, orthopaedic and medical wards.

Liaisebetween nurses, junior doctors, multi-disciplinary teams, patients, relativesand senior medical teams.

Attend andparticipate in ward rounds as timetabled.

Attend andcontribute to liaison activity in acute frailty, orthogeriatric.

Study forhigher examination where needed and maintain continued professional development.

Attendweekly educational and multidisciplinary sessions.

Participatein the teaching and training programmes for Junior Staff and PostgraduateMedical.

Tosupervise and manage junior medical staff depending on experience.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

From April 9th 2025 - Healthcare assistants - You must have a minimum of two yearsof experience working within the NHS as a HCA for the Trust to considersponsorship (we will only consider sponsorship if you meet this criteria andhave a current visa expiring within 3 months)


Job responsibilities

Ensurecontinuity of medical care for patients admitted through regular assessment andday to day management

Liaisewith professionals to promote appropriate admission, timely discharge andseamless care

Contributeto weekly multidisciplinary meetings

Bepotentially responsible for the professional and educational supervision ofjunior staff depending on experience

Beexpected to participate in the postgraduate teaching of medical staff andundergraduates attached to the Trust and in the training of all grades ofstaff.

Formanagement purposes and use of resources the post holder will be accountable tothe Clinical lead of Elderly Care, and will ensure the effective and efficientuse of resources allocated

ContinueProfessional Development (CPD) the post holder is responsible for ensuringthat they meet the national requirements for CPD, both internal and external

Beexpected to participate in clinical governance including audit arrangements inline with national and local guidelines and in respect of undergraduate andpostgraduate teaching in the Trust in accordance with current best practice

Beexpected to deputise in the absence of colleagues in cases of annual leave,study leave and sickness as far as is practicable and ensure continuous patientcare

If wishescan explore involvement in acute medicine on call.

Willbe supported locally for medical appraisal
